Passport Information
Passport Application Processing By Appointment Monday Through ThursdayRequirements To Obtain A Passport
- Original Birth Certificate (with raised seal) OR Original Certificate of Naturalization OR A previous U.S. passport
- (Females may be required to provide additional documents for name changes since birth.)
- Picture I.D.
- Adults: Driver’s License
Children: School I.D./Driver’s License- Two (2) checks (or money orders)
- 1- Payable to the U.S. Department of State
- $75 per applicant – 16 years of age and over
$60 per applicant – under age 16- 2- Payable to the City of Parkland
- $25 per applicant
- Two (2) passport photos – 2” x 2” – with a white background
Regular passports are valid for 10 years for individuals 16 years of age and over, and five years for passports executed before a child’s 16th birthday. Routine processing may take approximately 4 to 6 weeks; expedited service, which takes approximately 3 weeks, is available at an additional cost of $60 per applicant (excluding express mail fees).
Children Under 16 Years Of Age:
It is required by law that both parents appear along with their child to execute an application for a child under age 16. If this is not possible, the single applying parent must show proof of sole custody OR the consent of the second parent (through execution of Form DS-3053).
Evidence of the child’s relationship to parents or guardian must be provided (i.e., parents’ names on birth certificates, adoption decrees, etc.).

Print the forms and complete by hand OR complete them online, print them out, and then bring them with you. Save time at the passport office! If you have never before had a passport, use Application for a U.S. Passport (DS-11). If you have already had a passport, use Application by Mail (DS-82) – if issued within the last 15 years, you were 16 years of age or older when issued, your name is unchanged, and you can submit your previous undamaged passport.

by providing the last name as shown on the application, the date of birth, and the last four digits of the Social Security Number.
